It is an attempt to quantify the amount of tissue mass (muscle, fat, and bone) in an individual, and then categorize that person as underweight, normal weight, overweight, obese or morbidly obese based on that value. Bariatric Surgery Criteria

BMI > 35 with two or more co-morbidities

BMI > 40 with or without co-morbidities

Co-morbidities could be High Blood Pressure, Diabetes, Sleep Apnea, High Cholesterol, Chronic Back Pain or Joint Pain, etc.

Bariatric surgery is covered by most health insurances including Medicaid and Medicare.

One of the first thing you should do is call your insurance company and ask if you have bariatric coverage and what are the requirements.
